Periodontics is the dental specialty focused on the health of the periodontium — the collective term for all the structures that support the teeth within the jawbone. These structures include the gingiva (gum tissue), the periodontal ligament (the fibrous connective tissue that suspends each tooth root in its socket), the cementum (the outer surface of the tooth root), and the alveolar bone (the jaw bone that houses the tooth sockets). A periodontist completes two to three years of specialty training after dental school focused on diagnosing and treating disease of these structures.
Periodontists manage all stages of periodontal disease — from monitoring patients with gingivitis, through non-surgical scaling and root planing for moderate periodontitis, to complex surgical interventions for severe disease. Specific procedures include osseous surgery (reshaping bone to eliminate deep periodontal pockets), gum grafts (to treat recession and rebuild thin gum tissue), bone regenerative procedures (using membranes and growth factors to regrow destroyed bone), crown lengthening surgery (exposing more tooth structure for restorative purposes), and dental implant placement. Periodontists often co-manage complex cases with the patient's general dentist.
A referral to a periodontist is typically recommended when gum disease has progressed beyond what a general dentist can manage, when a patient has rapidly progressing bone loss, when non-surgical treatment has not produced sufficient improvement, or when surgical procedures are needed. Some patients also see periodontists directly for implant consultations. Signs that may prompt a referral include deep periodontal pockets (6mm or greater), significant bone loss on X-rays, gum recession requiring surgical correction, or persistent gum disease despite regular professional maintenance.
