Fluoride is a naturally occurring mineral โ a form of the element fluorine โ that has a well-established ability to strengthen tooth enamel and reduce the risk of cavities. It works through two main mechanisms: it incorporates into the crystal structure of enamel to make it more acid-resistant, and it inhibits the bacterial enzymes that produce the acids which cause decay. At the concentration found in water and toothpaste, fluoride is highly safe and effective.
Fluoride exposure comes from multiple sources. Community water fluoridation (adjusted to approximately 0.7 mg/L in the US) provides a constant low-level protective effect throughout the day. Fluoride toothpaste delivers fluoride directly to tooth surfaces at every brushing and is the single most impactful daily source for most people. Professional fluoride varnish, applied in the dental office, provides a concentrated topical dose that releases slowly over several hours. High-risk patients may also be prescribed fluoride rinses or gels for daily home use.
One of fluoride's most important properties is its ability to facilitate remineralisation โ the process by which minerals (particularly calcium and phosphate) are deposited back into slightly demineralised enamel, reversing the early stage of tooth decay before a cavity forms. This is why early white-spot lesions, which are areas of subsurface enamel loss, can resolve with fluoride exposure and improved oral hygiene. Once the enamel surface has physically broken down, remineralisation cannot rebuild it, and a filling is necessary โ making early detection and preventive fluoride use especially valuable.
